Emergency Room Services
My role with Continuing Care from 2017-18 was powerful and enlightening. I enjoyed working with the senior population, and learned so much. My role was as a geriatric resource coordinator (GRC) in the emergency department at SSRH.
With a "home first" philosophy, my role was to provide in-home support services to seniors, to avoid hospital admissions as much as possible.
Developed warm, friendly rapport with seniors waiting to be seen, as well any those in the waiting room.
Created easy to use visual guides for seniors to "shop for the services" they may require. I used best practices in accessibility in its creation.
Identified a significant gap in services in the ED regarding adults in need of AP, adult protection.
Created a role for myself as part of my GRC role to be the hospital liason for adults coming into hospital under Adult Protection.
Created position as a way to bring AP into the fold ASAP.
Saw through their care and placement until discharge from hospital.
